Medical Consent:
I agree that I/We will not hold E2E Elite and its representatives liable for any injury or accident that may occur to my child during practice, training sessions, or games. You are fully aware that there is a risk of injury involved while participating in this type of activity. I do now authorize the staff of the E2E Elite to consent on my behalf as they may deem necessary to assist my child with emergency medical care including ambulance transport and hospital care, as may be deemed necessary under the existing circumstances by available medical personnel. In doing so, I assume responsibility for all expenses of any such care not covered by my insurance.

Membership Rules:
If the player/family chooses to no longer participate in the E2E Elite program after participation in practice, tournament, or training session, they will forfeit all funds paid as well as 60 Days prior to the season. All refunds are approved by the program director only.
